MARPE – upper jaw expansion in adults
A narrow upper jaw can cause crowding, crossbite or a discrepancy between the widths of the upper and lower jaws.
In adults, conventional tooth-borne expansion is often less suitable because jaw growth is complete. In selected cases, MARPE can be used to achieve a more skeletal expansion of the upper jaw.

What is MARPE?
MARPE stands for Miniscrew-Assisted Rapid Palatal Expansion.
The treatment uses an expansion appliance anchored in the palate with small miniscrews. This allows more of the force to be transferred to the upper jawbone rather than only to the teeth.
MARPE is usually combined with fixed braces to correct tooth position and the bite after expansion.
Assessment and planning
Before treatment is planned, a clinical examination, photographs, a digital scan and radiographs are usually required.
A CBCT scan may be necessary to assess the available bone, anatomical conditions and suitable positions for the miniscrews.
The findings are used to determine whether MARPE is a realistic option or whether another treatment would be more predictable.
How does the treatment work?
The appliance is individually made and anchored in the palate with miniscrews.
After placement, the expansion screw is activated according to a specific schedule. A temporary gap may develop between the upper front teeth during expansion. This is normal and is corrected later with orthodontic treatment.
Once the desired expansion has been achieved, the appliance is usually kept in place for a period to stabilise the result.
Limitations and possible complications
The result cannot be guaranteed, and the response varies between adult patients.
Possible challenges include discomfort, irritation of the oral mucosa, loosening of a miniscrew, asymmetric expansion or failure of the palatal suture to open. In some cases, the treatment plan must be changed or surgically assisted expansion considered.
Good oral hygiene and regular follow-up appointments are important throughout treatment.
